How to Join DDS
To join Dental Directory Services, Please;

1. Get the agreement here. (PDF format)*

2. Print out the agreement.

3. Read, complete and sign agreement.

4. Fax the signed agreement back to us at

(516) 576-9268.

5. We will then contact you when we have added you to our directory.
